Squeeze Hand
This hand occurred March 24, 2006, in club play. The play, as described, occurred at two of the three tables I know of. Making 6 would have been a complete top.

The bidding was a simple 1NT, 3NT. The opening lead is a small diamond. Righty shows out, pitching a heart. You now test the waters in spades. Lefty plays the jack of spades on your king, and throws a diamond on the queen of spades.
The hand is now cold for making six, so you can claim -- but you have to describe the play.
